Continued VFR into IMC is rarely a single moment of bad judgment. It is usually a chain of small concessions: 'just a few more miles,' 'it looks clearer ahead,' 'we can descend under it.' NTSB timelines show that the fatal loss of control often occurs several minutes after the first objective sign that visual flight rules can no longer be maintained.
Decision-point analysis maps those concessions to identifiable triggers: entering a cloud layer, losing sight of the horizon at night, reaching a minimum safe altitude without visual references, or failing to locate an airport in marginal visibility. Interventions that work in training — explicit go-around triggers, minimum altitude buffers, and verbalize-if-you-lose-VFR drills — target these triggers rather than the final spiral.
